Forms will be sent out via email up to five days before your appointment.  Please complete this form as soon as possible so that we can ensure we are able to carry out your treatment safely

Cancellation Policy

24 hours notice is required to move or cancel an appointment.  We will charge 50% of the total cost of the cancelled appointment if 24 hours notice is not given.  For appointments over 1.5 hours long a 50% non refundable deposit will be taken 3 days in advance.  Late arrivals will result in your treatment being adjusted accordingly.

Please let us know if you have any medical conditions before you book an appointment with us.  

With certain conditions a doctors confirmation may be required to ensure we can safely carry out your treatment.
